I think they're using Gaiseric/Skull Knight as a mirror as to what Guts could potentially be if he keeps giving into rage. I felt they were foreshadowing a bit when the older witch-man was talking with Guts and Skull Knight, and asking Guts if he would let himself be consumed by his anger, or use it to keep himself alive.
The first, visually, is the image that comes to mind when I think of Guts at his angriest. The second, storywise, I think is the pinnacle of his rage (other than maybe what the Berserker armor invokes)
